REQUIREMENTS FOR BUSINESS PERMIT APPLICATION

(CERTIFICATE OF INSPECTION)

(Occupancy Permit)

NEW APPLICATION

     1. Barangay Permit to Operate Business

     2. Tax Declaration (OHA); Title

     3. Current Tax Receipt (Amelyar)

     4. DTI Clearance / SEC (for corporation)

     5. SSS Clearance

     6. Current Residence Certificate (Sedula)

     7. Valid Identification Card (ID)

Remarks: Prepare a photocopy of documents (1 copy)

 

REQUIREMENTS FOR BUILDING PERMIT APPLICATION 

(for small residential house/indigenous materials)(below 40 sq. m.)

     1. Barangay Clearance (specific for construction/renovation)

     2. Duly Notarized Authorization from the Lot Owner

            (if not registered owner of the lot)

     3. Tax Declaration (OHA), Title (if available)

     4. Current Tax Receipt (Amelyar)

     5. Floor Plan (in short/long bond paper)

     6. Current Community Tax Certificate (Sedula)

     7. Valid Identification Card (ID)

     8. Long Brown Envelope (1 pc.)

            (all documents must be 3 copies each)

 

REQUIREMENTS FOR ELECTRICAL PERMIT APPLICATION

(for small residential house/indigenous materials)(below 40 sq. m.)

     1. Barangay Clearance (specific for electrical installation)

     2. Duly Notarized Authorization from the Lot Owner

           (if not the registered owner of the lot)

     3. Tax Declaration (OHA), Title (if available)

     4. Current Tax Receipt (Amelyar)

     5. Current Community Tax Certificate (Sedula)

     6. Valid Identification Card (ID)

     7. Long Brown Envelope (1 pc.)

     8. Yellow Card (from MERALCO)

          (all documents must be 3 copies each)

 

 REQUIREMENTS FOR SECURING BUILDING PERMIT

(above 40 sq. m.)

    1. In case the applicant is the registered owner of the lot (3 copies)

          a) Certified Xerox copy of Transfer of Certificate of Title (TCT or OCT)

          b) Certified Xerox copy of Tax Declaration (OHA)

          c) Current Real Property Tax Receipt (Amelyar) or Certificate of Non-Tax Delinquency

     2. In Case the applicant is not the registered owner of lot. (3 copies)

           a) Duly notarized copy of the Contract of Lease / Deed of Absolute Sale / Contract of Lease / Deed of Absolute Sale / 

              Contract of Sale / Authorization from the Lot Owner

     3. FIVE (5) sets of plan and specification prepared, SIGNED AND SEALED.

         Note: Use STANDARD SIZE of Blue Print (20" x 30")

           a) Architect or Civil Engineer for Architectural and Structural Portion.

                   NOTE: Provide minimum requirements for Accessibility as required by B.P. 344 Accessibility Law

                   (a. ramp     b. parking    c. toilet)

           b) Sanitary Engineer or Master Plumber for not more than 20 units of plumbing fixtures for 

               Sanitary Portion.

          c) Professional Electrical Engineer or Master Electrician for not more than 4 kilowatts load for Electrical Portion.

          d) Professional Mechanical Engineer for Mechanical Portion (if any)

                           * Clear Xerox copy of Current PTR and PRC ID of Design Professionals. (1 copy)

                           * Need Approval from the Developer (if inside the subdivision)

     4. Forms (Building; Sanitary; Electrical; Fencing; others

     5. Bill of Materials / Bill of Quantities and Specifications (5 copies)

     6. Fire Safety Permit (to be secure at the Chief of Local Fire Service upon endorsement by the Building Official)

     7. Locational Plan of duly prepared, signed and sealed by a licensed Geodetic Engineer (5 copies)

     8. Mayor's Permit/Locational Clearance for MHS.

     9. Structural Design and Computation/Seismic Analysis for Structure of more than 7.5 meters in height.

            - Clear Xerox copy of Association of Structural Engineers of the Philippines (ASEP) membership Valid ID (1 copy)

     10. Approved Lot Plan / Cadastral Map / Logbook (2 pcs.)

     11. Barangay Clearance (specific for Construction / Renovation) (3 copies)

     12. Clearance from DPWH (District Office) if along National Road (1 copy)

     13. Clearance form PEO (Provincial Engineer's Office) if along Provincial Road (1 copy)

     14. DOLE Certificate of Construction Safety & Health Certificate (1 copy)

     15. Notarized Authorization of Representative / SPA (3 copies)

     16. Valid I.D. of applicant / lot owner / representative (2 copies)

           Others: Birth Certificate / Marriage Contract / Death Certificate (2 copies); CTC (1 copy)

           Special Retirement Residence Visa (SRRV); Alien Certificate of Registration (ACR) - if foreigner (1 copy)

     17. Long Expandable Envelope

               Red - Residential (New Construction)                                        Green - Institutional

               Orange - Residential (Renovation / Extension)                           Violet - Cell Site

               Yellow - Commercial                                                               Brown - Fencing

               Blue - Commercial (Poultry / Piggery)                                       Pink - Site Development

 

Note: - Applicants shall sign on all set/s of plan/s, specification/s and bill of material/s.

         - Permit Forms issued at this office should be completely and legibly filled-up

         - Other requirement/s may be required if deemed necessary.
